hospital discharge CHC test

My friend ended up in hospital, it's the one stuck in the middle needs more care but doesn't qualify for more care, the extra care is supplied by relatives/neighbours popping in.

The hospital who were quick to discharge as care is in place, not being sent home to an empty house.

The discharge nurse agrees with the relatives, that this lady needs more care and should not be on their own for 48 hours after discharge. But there is only the care package 3 carers a day, majority of the time lady is on her own.

With a lot of badgering, the nurse is trying to get a continuing health care assessment but shouldn't this have been done before discharge.
The social Services are not willing to increase care but have come up with a wacky idea which suits no one.
There is a place in a sheltered housing scheme miles away and actually not in the same county.
So then the Social services would be passing the responsibility onto another authority.

How would that even work, my friend would have to be reassessed by this new authority and a new care plan set up.

My friend wants more care in their own home, thats what CHC is all about is it? but the hospital say that CHC will not pay for 24 hour care as my friend is not bad enough.

So how does this CHC work, how long does it take to do the assessment and sort out the extra care, days/weeks.
Thats if they pass and if they don't what then?

All agree social services, the social worker is actually quite sympathetic and agrees that more care is needed, the relitives agree, the hospital agrees, the neighbours agree and so do i.

Perhaps the first local authority has an arrangement with the second local authority to provide sheltered housing in the other district.
