Just a quick question.

One of the staff from the temporary care agency I employ for personal care has entered a statement in the written notes which was false and damaging to my mother. The care agency now wishes to remove the document from the premises but what about if it gets lost or they alter it in some way? I have taken a copy but can I insist it stays within the property as it's my only form of evidence.

If they supplied the actual paper then it belongs to them. Before handing it over, get an independent, named witness to countersign and date the photocopy as a true facsimile record.

I have on a couple of occasions immediately corrected the record by recording what was said/happened and signed my entry, that way there is a contemporaneous record of both versions should there be problems, in your case where the record is to be removed I would also do what Excalibur suggests as well.
