Hidden reserves

For issues specific to caring for someone with dementia.
Just visited Dad , and doesn't look like he's eaten or drunk much in the last couple of days, his legs are skeletal but towards the end of my visit he woke up and asked for a match followed by extremely LOUD vocal requests for help, he wanted a smoke etc. etc.- He gave up smoking in the early 1980s. How can someone so frail and short on energy make so much noise?
